Answer one question at a time; the incomplete Kawasaki pathway opens when needed. Tap any earlier answer to change it.
This tool walks clinicians through the diagnosis of complete and incomplete Kawasaki disease in children, one question at a time, and gives treatment and follow-up steps when criteria are met. An optional weight entry converts doses to milligrams.
It starts with fever duration: 5 days or more, 4 days with 4 or more principal features, or 7 days or more in an infant under 6 months with no other cause. It then counts the five principal features: conjunctival injection without discharge, oral changes, rash, hand and foot changes, and a cervical node of 1.5 cm or larger. Four or five features mean complete Kawasaki disease. With 2 or 3 features (or in the young infant pathway), the incomplete algorithm checks for CRP 3.0 mg/dL or more or ESR 40 mm/h or more, then for 3 or more supplementary lab findings or a positive echo.
Results range from serial evaluation or an echocardiogram to treatment with IVIG 2 g/kg and moderate- to high-dose aspirin, followed by low-dose aspirin 3–5 mg/kg/day. It also lists echo timing, high-risk features and IVIG resistance steps. The content follows the AHA 2017 scientific statement on Kawasaki disease (McCrindle et al., Circulation 2017).
In this tool, a child with qualifying fever and 2 or 3 principal features (or a young infant with prolonged fever) is assessed with CRP 3.0 mg/dL or more or ESR 40 mm/h or more. If markers are high and there are 3 or more supplementary lab findings or a positive echo, treat as Kawasaki disease.
IVIG 2 g/kg as a single infusion over 10–12 hours, ideally within 10 days of fever onset, given with moderate-dose aspirin 30–50 mg/kg/day in 4 doses (80–100 mg/kg/day is also used) until afebrile for 48–72 hours, then low-dose aspirin 3–5 mg/kg/day, max 81 mg.
The tool recommends an echo at diagnosis, at 1–2 weeks and at 4–6 weeks. Low-dose aspirin continues until the echo is normal at 4–6 weeks.
